A support coordinator acts as your professional advocate and logistical manager. We explain your complex plan rules, identify appropriate therapists or community networks, coordinate service agreements, and resolve delivery issues on your behalf. Our ultimate goal is to build your confidence so you can eventually manage your own supports.
To access this service, you must have an approved budget allocation for "Support Coordination" specifically listed under your Capacity Building budget. If it is not currently included, you can request an unscheduled plan review through your Local Area Coordinator (LAC) if your management circumstances change.
We maintain active, collaborative communication with your entire support network. With your written consent, we organize clinical case reviews, coordinate feedback with occupational therapists, speech pathologists, and counsellors, and ensure every practitioner is aligned with your core NDIS plan outcomes.
